Newborn alive, baby boy via Vacuum Extraction for Fetal and Maternal Distress (cord round neck and body + LMSL (light meconium stain liquor) with 2 previous LSCS. Cord clamped by Dr. D, baby tagged by SN K. Apgar Score 9/10. Skin pinkish, all limb move well, cried well. Wt: 3333gms, length: 56cm, HC: 36cm. Oral and nasal suction, top+tail, full examination done. No abnormalities noted. Anus pattern. Baby NPU and BO. IM Vit. K 0.1ml given at left rectus femoris and IM Eng. B 0.5mls given at right rectus femoris by SN K at 0630hrs. Azan and breastfeeding done. To transfer out baby to nursery.
